ITC (Integrated Team Care Program) is a program that supports our mob to stay healthy and manage long term health problems.
ITC is a program for our Aboriginal Clients with a Chronic Disease.
ITC ensures you are on top of your healthcare by helping with seeing GP's & Specialists, getting to your appointments and supporting your healthcare.
Clients can join our ITC program by visiting one of our GP's and asking about a Care Plan.
IPTAAS
IPTAAS (Isolated Patients Travel and Accommodation Assistance Scheme) is another program designed to help our mob get specialist health care wherever it may be located.
IPTAAS can be accessed by any of our clients who have a referral for specialist care that is over a distance away. The IPTAAS program can offer reimbursement for both travel and accommodation. All it takes is filling out a form!
